Approval
A human or rule-based decision that gates a specific gateway operation before it runs.
Approvals are governed by per-operation risk tiers (low, medium, high), a user-configured defaultApprovalTier (default high), and scoped alwaysAllowRules[] with default 7-day TTL. The cloud can force an approval with x-desktop-force-approval: 1.
Decisions resolve to approved | denied | always_allow | expired.
